Plan the work, then work the plan.
Weak willpower, poor dedication, lack of commitment. These are definitely all factors that keep millions of people from reaching their goals, but I wouldn't say any of these are the most common reason for failure. In my opinion, a lack of planning is why most New Years' resolutions don't last beyond January. A well-devised plan seems to decrease the willpower, dedication, and commitment needed by making the process part of a normal routine.
It's the difference between a resolution to work out 3 days a week and deliberately scheduling to go to the gym before work on Mondays, Wednesdays, and Fridays.
It's the difference between the guy who wastes time debating what to do in the gym and the guy who walks in, quietly knocks out his lifts, and goes on about his business.
Of course, not all plans are created equal and no program is right for everyone. It's important to choose one that fits your specific goals and obligations. However, any decently formulated routine will undoubtedly produce better results than a random attempt to get in shape. A systematic routine allows you to track progress and ensure consistency as opposed to simply choosing a new lift every day. Here are a few very different but proven routines that I would recommend:
Bigger, Leaner, Stronger by Mike Matthews
MAPS by Mind Pump
Starting Strength by Mark Rippetoe
